In checking the latest premium payment, we have discovered that {{surrogate_full_name}}'s policy has cancelled for nonpayment. Please let us know as soon as possible if this policy is indeed supposed to be cancelled as it is no longer needed, or if the policy needs to remain in place. If it needs to remain in place, we would try to reinstate it and if approved, a payment will need to be made.
Please Note: If all 3 notices for premium bill monitoring are sent out and the ACA policy is terminated/cancelled, by the insurance carrier (due to the premium payment not being made in a timely manner) the following will apply;
