Without your signature we will be unable to submit your cancellation request to the insurance carrier, and there may end up being another month of premium payment due. We appreciate your prompt assistance, please reach out to us with any question or concerns.</span></p><p><br></p><ul><li>Please note there may be additional steps that the Gestational Carrier may be required to complete in order to finalize this cancellation request. If the Gestational Carrier's assistance is not provided in a timely manner, the member may owe an additional monthly premium.</li><li>Cancellations requests for the current month, must be received by the 18th of the month.<br></li><li>The signed cancellation document from the Gestational Carrier needs to be returned no later than the 20th of the month.</li><li> If cancellation request is received on or after the 19th, the cancellation instructions would be sent to Gestational Carrier. We can process inhouse but will be processed on the following month. Which could mean an extra month of premium being due.</li><li>Once a cancellation is processed with the insurance carrier, it is hard to undo the cancellation.
