I will send {{surrogate_full_name}} a cancellation form via the Client Portal, that is necessary for her to sign in order for us to proceed with the cancellation. Once we receive the signed document, we will begin the request for cancellation of her policy. Cancellations are time sensitive so the sooner the document is completed, the better.
Please Note: The policy must be paid through the end of the month in which you are requesting a cancellation. If the policy is not correctly cancelled, you will be responsible for subsequent premiums.
