Please contact her regarding any questions or concerns regarding your application status and any premium billing concerns at any time during your journey.</p><p><br></p><p><b><font color="#ff0000">In order to finalize your enrollment with our office, we need the following.</font></b></p><ul><li><b><font color="#ff0000">A copy of your insurance ID card</font></b></li><li><b><font color="#ff0000">Signed HIPAA</font></b></li><li><b><font color="#ff0000">Broker of Record form (if needed)</font></b></li></ul><p><b><font color="#ff0000"><br></font></b></p><p><b><font color="#ff0000">Once you have received your member ID from the Insurance Carrier, please contact Melissa with your member ID. You may also text it to us at 661-491-7707. If you create a member login with your Insurance Carrier's portal, please provide us with your login information. </font></b></p><p><b><font color="#ff0000"><br></font></b></p><p>We will be reaching out in the near future via your ART Risk portal to have you complete a HIPAA and, if needed, a broker on record form. This HIPAA is for ART Risk to obtain information on your policy in order to confirm the status and coverage with member services. The Broker on Record form is for us to be the named agent on the application and be able to call broker services.
