Subject <p> URGENT ACTION NEEDED – RISK OF POLICY CANCELLATION! {{case_id}}</p>

Body <p class="MsoNormal">Hello {{agency_name}},</p><p class="MsoNormal"><br></p><p class="MsoNormal">We have sent two prior email reminders that there are unpaid premium payments in your Agency Portal under Premium Bill Monitoring and the policy for {{surrogate_full_name}} is now at risk of cancellation. We highly advise immediate action of the following steps to remedy this issue and ensure that all steps are followed so that the policy does not cancel. As a reminder, if a policy cancels for non-payment, it will most likely <i>not get reinstated by the insurance company</i>, and the Gestational Carrier could be without insurance coverage for the remainder of her journey. </p><p class="MsoNormal"><br></p><p class="MsoNormal">We ask that you take the following steps to avoid cancellation:<span style="text-indent: -0.25in;"> </span></p><ul><li><span style="text-indent: -0.25in;">Log into the GCs insurance portal and submit payment immediately. All payments should be made by the last day of the current month.</span></li><li>Take a screenshot of the record of payment in the insurance portal. If there is a confirmation number, provide both the screenshot and confirmation number to your ART Risk Premium Bill Monitoring Specialist for our records. We will then verify the insurance carrier correctly applies the payment made.</li><li>If you have already made payment (prior to this notice) and the insurance carrier is not showing record of this, confirm with the bank or credit card company that: a.) the payment processed; b.) the date the payment processed; c.) a confirmation number if provided and/or confirmation email from the insurance carrier. Provide all the noted information to your Premium Bill Monitoring Specialist and we will contact the insurance carrier and make sure the payment is correctly applied to the GC's account.</li></ul><p class="MsoNormal"><br></p><p>Please Note: If all 3 notices for premium bill monitoring are sent out and the ACA policy is terminated/cancelled, by the insurance carrier (due to the premium payment not being made in a timely manner) the following will apply;</p><ul><li style="margin-left: 25px;">Gestational Carrier must contact the insurance carrier/member services and ask about the possibility of reinstating the policy, how much is due, etc. If they will allow reinstatement, prompt payment needs to be made to ensure it is actually reinstated. </li><li style="margin-left: 25px;">We can assist with the reinstatement, however, please understand it is a time-consuming task and may require the Gestational Carrier’s cooperation. A reinstatement, is not part of our premium bill monitoring service, so our hourly fee will apply. ($150 per hour/2 hour minimum).</li><li style="margin-left: 25px;">If policy can be reinstated, we will continue to check the upcoming months, for which the premium bill monitoring service is paid for.<br></li></ul><p style="margin-left: 25px;"><br></p><p>Thank you for your prompt attention to this urgent matter.
