We will need this form completed in order to move forward. You can disregard this task if you submitted a payment authorization form upon submitting your request or if we already have a payment method on file.</li><li>Once the credit card authorization form is completed and received by our Team, we will begin the process of confirming in-network providers, vetting ACA policies that will work for your Gestational Carrier, and providing you that information once completed.</li></ul><p><br>Again, thank you for the opportunity to assist with this aspect of your journey.
