Policy Validation: Missing Booklet {{case_id}}
Hello {{agency_name}},The booklet that was uploaded with the request for {{surrogate_first_name}} is not the correct booklet. The book is usually 60-100+ pages and contains information regarding covered benefits, exclusions, coordination of benefits, etc. It is frequently called an Evidence of Coverage or Summary Plan Description.

Please note, the policy booklet is needed in order to start the policy validation. Due to time constraints, we are not able to search for policy booklets. Thank you for your understanding.
To upload the booklet needed, please login to your ART Risk agency portal, open the Case and navigate to the Operations tab. Here you can upload the required booklet by clicking the green '+ Add Files' button. Please use document type 'Evidence of Coverage'. Once you have uploaded your document, click on the Products tab on the Case and mark the 'Agency: Upload Missing Booklet' task complete under the Policy Validation product. This will alert the Policy Validation team that the document has been uploaded.
