Body <p>Hello {{agency_name}},</p><p><br></p><p>Thank you for submitting the policy validation request for {{surrogate_first_name}}. As the Evidence of Coverage booklet does not outline specific deductible, out-of-pocket, and co-insurance amounts, we will also require the Summary of Benefits (also called Schedule of Benefits) document. This 4–10-page document is readily available on the members portal. <br><br></p><p>To upload the summary of benefits needed, please login to your ART Risk agency portal, open the Case and navigate to the Operations tab. Here you can upload the required booklet by clicking the green '+ Add Files' button. Please use document type 'General Document'.
