Body <p>Hello {{surrogate_full_name}},</p><p>An insurance policy for your surrogacy journey has been selected by your Intended Parent, {{ip_full_name}}, and ART Risk will be contacting you to confirm general information and schedule your insurance application appointment.  Here are the next steps:</p><ul style="margin-top:0in" type="disc"> <li class="MsoNormal" style="mso-list:l1 level1 lfo1;tab-stops:list .5in">Within the next few days, we will be calling you from a 661-area code number and/or texting you from our texting app (661) 438-0270 to get you scheduled with one of ART Risk's application agents who will submit your application with the Insurance Carrier.  The application appointment will be completed over the phone.</li></ul><p><br></p><p class="MsoNormal"><strong>As this is a do not reply email, do not respond to this email verifying your information. Our scheduling team will reach out to you.  </strong>Our scheduling team will need to verify the following information is correct before we schedule you to apply:<br>-Legal first and last name<br>-Date of birth<br>-Address and county<br>-Phone Number<br>-Email<br>-Preferred OB (if one was noted on the request)</p><ul style="margin-top:0in" type="disc"> <li class="MsoNormal" style="mso-list:l0 level1 lfo2;tab-stops:list .5in">If you reside in California or Colorado, ART Risk will need a copy of the front and back of your driver's license. You can send this through our texting app at (661) 438-0270.<br style="mso-special-character:line-break"> <!--[if !supportLineBreakNewLine]--><br style="mso-special-character:line-break"> <!--[endif]--></li> <li class="MsoNormal" style="mso-list:l0 level1 lfo2;tab-stops:list .5in">If you are responsible for the first month's premium (binder payment), then ART Risk will need to verify you have received the funds and are prepared to make the payment at the time of your scheduled appointment.  <strong>Your agency and/or Intended Parents should be communicating with you if the binder payment (i.e. first month's insurance premium) will be required during your application appointment.  If they have not confirmed this information with you, please reach out to them immediately.</strong></li></ul><p><br></p><p class="MsoNormal">The insurance carrier SHARP has been selected for your health insurance policy for your surrogacy journey. Please note, for SHARP, we will need two proofs of residency on file before your application appointment.  Proofs must clearly state your full name, address, and should be the most recent version of the document available. Some acceptable residency documents include: Gas, Electricity, or Water utility bills; or Internet billing statement, bank statement, California driver's license, rental agreement, school records, paystub, tax return, Property Tax statement. Please text photos of the two proofs before your application appointment through our texting app at (661)438-0270. Your name and address must be included on the proofs. If we do not have these, you will not be able to complete your insurance application.</p><p class="MsoNormal">Confirming the information above is imperative to applying you successfully for insurance. Failure to confirm this information could result in delayed coverage.
