For both policies, the effective date of the policy is the medication start date of the cycle. This policy is purchased per cycle, so if another cycle commences within the coverage period, a new policy must be purchased. The policy covers complications that may arise from the medication, retrieval of egg(s) or transfer of the embryo (depending on the coverage selected). Policy does not cover the actual medication or procedure costs. The issued ID card must be presented at the time of service.
Multiplan Network (multiplan.com) in order to receive the maximum benefits of the New Life Agency policy, in-network providers should be used. Please click on the link to look up in-network providers. Co-insurance applies for out of network providers. In an emergency situation, an out of network hospital would be treated as in-network. Hospitals have to pre-authorize inpatient stays and/or procedures, as long as the ID card is presented this should not be an issue.
** An ectopic pregnancy would be considered a complication. A miscarriage is not considered a complication.
The likelihood of there being complications is low. If your surrogate has insurance that covers IVF, then if a complication occurs, her insurance should cover.
This policy is intended for cases in which the surrogate has no insurance, waiting for insurance to be active and has no current coverage or if surrogate\'s personal insurance is questionable in covering complications from fertility medication/transfer of embryo.
If you have any questions, please contact us.
Warm Regards,
ART Risk Team
