Please see the attached product information and pricing information for New Life Agency (NLA) and for Universal Family Insurance (UFI). The first 2 documents are for NLA the other 2 documents are for UFI. This email contains a copy of policy for both carriers. The comparison below is for donor only coverage.

For both policies, the effective date of the policy is the medication start date of the cycle. This policy is purchased per cycle, so if another cycle commences within the coverage period, a new policy must be purchased. The policy covers complications that may arise from the medication, retrieval of egg(s) or transfer of the embryo (depending on the coverage selected). Policy does not cover the actual medication or procedure costs. The issued ID card must be presented at the time of service.
Multiplan Network (multiplan.com) in order to receive the maximum benefits of the New Life Agency policy, in-network providers should be used. Please click on the link to look up in-network providers. Co-insurance applies for out of network providers. In a true emergency situation, an out of network hospital would be treated as in-network. Hospitals have to pre-authorize inpatient stays and/or procedures, as long as the ID card is presented this should not be an issue.
When the policy is purchased, you will receive a proof of purchase email that contains the ID card. You can forward the ID card to donor (with NLA you also receive a welcome letter to assist guide donor).
